If you've got a new Pokémon (excluding receiving your Starter Pokémon from the Yew Laboratory), it must be registered here to prevent the use of cheating. You must also link back to where you obtained it, also preventing cheating.

You must also post your Pokémon if it has evolved. Lay it out like this:

Pokémon | Gender
[Where Received]

You don't need to format it exactly like that, but as long as all three pieces of information are clearly visible, it's fine. Here's an example of a working format (the link just links back to the forum):
